New Patient Information
You will receive a text and email with a link to complete your New Patient Information. Please complete this as soon as possible in preparation for this appointment.
Our Dentists are thorough in your Dental Assessment, and we have a detailed form we require prior to the commencement of your appointment.
Please Email Us your completed form by the Day Prior to your Appointment. You will shortly receive a Welcome Email with a link to this form and you can simply reply to that email with your completed form.
If your appointment is made with short notice, please allow 20 minutes to arrive early and complete your form prior to your Appointment.
If you are attending for a New Patient Check-up and Clean – then this offer gives you great value. It includes a Basic Clean. This basic clean is what is considered an ‘Above the Gums Cleaning’ of your teeth. If you have Periodontal Disease, Moderate-Severe Tartar buildup, or sensitive teeth during cleaning then you will require additional cleaning appointments at additional costs. This additional cleaning would be ‘Below the gum-line’ cleaning.
